Stacy awoke with the worst sore throat she had ever had. She had been feeling tired for the past few days, but now she felt feverish and the glands in her neck were swollen. She felt too sick to get out of bed. For the next few days, her throat continued to feel sore and her glands were swollen and tender. Every afternoon, her temperature would rise to 101°F, and she felt exhausted even though all she did was sleep. Stacy decided to see her doctor, who saw that Stacy’s tonsils were swollen (Figure 1.1) and her throat was very red. Stacy’s heart and lungs sounded normal, and she did not have a rash. The doctor noted that Stacy’s abdomen was a little tender and could feel that her spleen was somewhat enlarged. Suspecting that Stacy had a classic case of infectious mononucleosis, the doctor took a blood sample to confirm the diagnosis. Infectious mononucleosis (mono) is a disease caused by the Epstein- Barr virus (EBV). EBV infects the cells of the throat and the immune system. The blood of someone with mono contains high numbers of white blood cells called mononuclear cells. Mononuclear cells also make the tonsils and spleen swollen. A person with a swollen spleen must avoid any extra pressure to the area; even a moderate blow can cause it to rupture. The immune system causes fever and fatigue as it attacks the virus. Antibiotics are not effective against viruses, including EBV.
