When the first edition of this book was published, psychiatry of old age in the UK was in its infancy. Old age services were not available in many health districts, and the number and range of professionals who were interested in providing services was limited. For example, there were only just over 100 consultant old age psychiatrists, whereas now there are over 500 and there has been a revolution in the provision of services. Treatment options have also been extended, with a vast increase in the choice of antidepressant, antipsychotic and, most recently, antidementia medication. The management emphasis of the NHS has changed, with a shift away from an unhealthy preoccupation with costcutting.
Now clinical governance gives equal emphasis to continual improvements in service, with managers having responsibility for the quality of clinical services as well as for balancing the books. In many places there has been an increased commitment to multidisciplinary, multi-agency working. The Audit Commission's Forget Me Not report and the new National Service Framework for Older People give official impetus to this approach. The emphasis is on working together, involving primary and secondary care, and health and social services. We all need to collaborate to address the complicated health and social problems that are experienced by older people. Because of the increasing number of old people in the population, and because of the tendency for disease and disability to become more common with increasing age, health and social services find much of their resources appropriately directed to this ageing population.
